REGISTRATION
Registration, along with paid fees, is required to attend any of the PHYSOR-2004 technical or plenary sessions. Two technical workshops will also be held on the days before and after the meeting. Although there are no additional fees for these workshops, registration and paid fees for the PHYSOR meeting are required to attend either workshop. An identification badge, provided at check-in, must be worn when attending sessions.
Meeting registration can be completed using our on-line registration process, which also includes secure payment options. Walk-in registration will also be available at the meeting. Information for hotel reservations is available here.
Full registration for the four-day PHYSOR-2004 meeting is $800 (U.S.). Members of co-sponsoring organizations will receive a $150 discount; please be prepared to provide membership identification at check-in. An additional $100 discount is given if registration is completed (including paid fees) on or before April 2, 2004.
Included with the full registration are
□ The meeting proceedings provided on a CD-ROM.
□ One (1) ticket to a welcome reception that will be held at the Art Institute of Chicago on Sunday, April 25.
□ One (1) ticket to each of the luncheons held on Monday and Wednesday (April 26, 28).
□ One (1) ticket to the evening banquet held on Tuesday, April 27.
Additional copies of the proceedings or tickets to the special events may be purchased when you register. One-day and student registration options are also available.
Please be aware of our cancellation policy. Registrations cancelled before April 2, 2004 will be refunded, less a $150 cancellation fee. We are sorry, but there is no refund for cancellations received after April 2. You may, however, send a substitute. Please promptly inform our registration staff of cancellations or substitutions.
